Job Title: Star of Hope Case Manager – Men’s Recovery Home

Job Summary:
The Star of Hope Case Manager is responsible for case management of men in early recovery and residing in Star of Hope. Needs to be able to relate and identify with the population being served, facilitate meetings, manage house responsibilities for the residents and implement house rules and regulations. Transportation assistance is arranged by this position for substance abuse treatment, medical concerns, and to other required services as needed. Referrals and collaboration with other area agencies and services are essential.
Responsibilities:
• Embrace, support and implement Community Healing Center’s vision and values related to diversity, equity, and inclusion.
• A thorough knowledge of community resources and men’s issues.
• Work with the Recovery Housing Supervisor in program development.
• Interact with clinical staff and other referral agencies regarding client progress.
• Interview and screen applicants for appropriateness for program.
• Perform admissions, intake and discharge functions as directed and according to policy/procedures.
• Document client interactions, referrals, activities and plans objectives in case management files.
• Complete GPRA and ARMS assessments.
• Efficiently schedule case management with clients and transportation assistance.
• Monitor client’s movements on recovery logs, chore list and sign-out board, etc.
• Mediate situations regarding participants. Maintain a therapeutic environment.
• Perform random drug or breathalyzer test.
• Community and program outreach as required.• Client advocacy with courts, other agencies.
• Problem solving with clients and appropriate response to crises situation.
• Assistance in obtaining basic needs, esteem building, basic life skills, health needs including those of the children such as immunizations and any special needs.
• Timely completion of progress reports, screening for eligibility, assessments, case management agreements, and other documentation as needed.
• Provide proper documentation to Recovery Specialty Services Supervisor for house census and billing.
• Maintain petty cash, purchase necessary supplies for household, and coordinate with supervisor any repairs or improvements needed.
Required Qualifications:
High School Diploma Required; Bachelor’s Degree preferred
Covid-19 and Flu vaccinations, required. Covid-19 booster, preferred.
Required Job Knowledge and Skills:
Additional education or experience in social work, human services and/or substance abuse treatment.
Additional Licenses or Certification:
• CPR, Infant CPR, First Aid are needed and a requirement for above
trainings.
• Reliable Transportation, Car Registration and Insurance filed with
Human Resources.
• Valid Driver’s License.
